All students wishing to enroll in Lawrence Virtual School (LVS) are required to provide health information to Lawrence Public Schools. Please complete the forms below as they pertain to your situation and submit prior to July 31.


Health Requirements for New Students

New students are required to submit specific information to Lawrence Public Schools. Please print the Health Requirements for New Students form, sign acknowledging your understanding of the requirements, and submit the completed form to Lawrence Virtual School, 2145 Louisiana St., Lawrence, KS 66046, prior to July 31. Enrollment is not complete until required forms are received by LVS office.


Physical Examinations

All students ages 9 and under who are entering a Kansas public school for the first time are required to have a Medical Examination form on file at their school. Please print the Medical Examination form, take with you to the physician to complete and sign. Please send completed Medical Examination form to Lawrence Virtual School, 2145 Louisiana St., Lawrence, KS 66046, prior to July 31. Medical examinations are NOT renewed annually.

Health History 

All students must have a Health History on file. Please download and print the Health History, fill it out, and mail it to Lawrence Virtual School, 2145 Louisiana St., Lawrence, KS 66046, prior to July 31.

Returning family: if there are changes in your child(ren)'s health, please fill out a new Health History and return it to the school. 

Religious Exemption from Immunizations

Parents exempting their child(ren) from immunizations for Religious reasons are required to write a statement for each of their child(ren) stating the child's name and what immunizations they will NOT be receiving for religious reasons. The parent will need to sign and date the letter.


Medical Exemption from Immunizations 

Students who are Medically Exempted from immunizations are required to have their physician fill out and sign the Kansas Certification of Immunization (KCI) form B Medical Exemption for inclusion in their school file.
